Basic Personal Information Needed: 

  • Valid State Issued ID for Taxpayer and Spouse (if applicable)

  • Social Security Card (taxpayer, spouse, dependents)

  • 2022 & 2023 Tax Returns (business and personal)

  • ID Pin for each individual filing (if applicable)

  • Proof of Dependency (Birth/Adoption Certificate, School Record, etc)

  • Voided check for Direct Deposit if applicable

  • 1095A form from the Healthcare Marketplace

Income Information :

  • W-2 Forms (income from employment)

  • 1099-NEC (nonemployee compensation)

  • 1099-DIV (income from dividends

  • 1099- INT (income from interest)

  • 1099-R (retirement or IRA payouts)
